Please submit the following information in electronic form to email@example.com by the due date listed in your award letter. Please include in the subject line of the email the grant number, your organization name and the program name.
1. Financial Information
a. Actual program revenues and expenses (six-month) versus the original program budget submitted in the proposal.
b. Discussion of any major budget variances.
c. 2009-2010 organization budget showing budget to actuals.
2. Program Update
a. An overview of program implementation to date. Include the number of clients served and, if appropriate, number of events/sessions, etc.
b. Summary of significant changes, if any, made in your program plans including: projected outcomes, timeline, evaluation plans, etc. and why these changes were made.
c. Progress to date toward the projected outcomes stated in the outcomes chart from your proposal (please report outcomes information using the chart format—see below).
3. Attribution Information
a. Description and documentation of how the organization has achieved the JCF attribution requirements described in the JCF award letter.
4. Marketing Material
a. Any marketing material not previously submitted.
