Final Report
Please submit the following information in electronic form to submissions@sfjcf.org by the due date listed in your award letter.  Include in the subject line of the email the grant number, your organization name and the program name.

  1. Financial Information
    1. Actual program revenues and expenses (year-end) versus original program budget.
    2. Discussion of any major budget variances.
    3. Final 2009-2010 organizational budget
  2. Program Outcomes
    1. A report on the actual program outcomes achieved in comparison to those described in your proposal. Please report the outcomes information using the chart format provided below
    2. A brief description of what the organization has learned from the program’s successes and challenges, and how the organization has or will apply this information to its work.
